A common misunderstanding: DHI and FUE are often described as entirely different techniques. They are not. The extraction phase — removing individual follicular units from the donor zone — is identical in both. The same motorised micropunch tool, the same graft storage protocol, the same donor zone management principles.

The difference is entirely in the implantation phase.
FUE implantation (two steps): After extraction, the surgeon opens channels in the recipient area using a blade or needle. Grafts are then implanted into these pre-made channels. The two-step process allows the surgeon to plan the entire recipient area before placing a single graft — advantageous for complex cases requiring dense coverage across large areas.
DHI implantation (one step): The Choi implanter pen combines channel creation and graft placement into a single motion. The follicle is loaded directly into the implanter and placed in one movement. No pre-made channels means less trauma to surrounding tissue, and the angle and direction of each follicle can be controlled at the precise point of implantation.

One of the most clinically significant advantages of DHI is the ability to perform the procedure without shaving the recipient area. The Choi pen can navigate between existing follicles without requiring shaved access.

This makes no-shave DHI the technique of choice for: professionals who cannot take time off with a visibly shaved head; women who need to return to work within days; and patients having a small touch-up or density improvement around an existing transplant.
The tradeoff: no-shave procedures are more time-consuming, technically demanding and typically limited to smaller graft counts (1,000–2,500 grafts).
For many patients — particularly those at Norwood IV with significant frontal and crown loss — the optimal plan combines both techniques. FUE is used for the main coverage zone where density across a large area is the priority. DHI is then used for the frontal hairline where follicle angle precision and density are paramount.

DHI (Direct Hair Implantation) uses a Choi implanter pen that creates a recipient channel and places the graft simultaneously in one motion. Unlike FUE where channels are pre-created, DHI combines these steps, offering superior angle control and enabling placement between existing hairs without shaving.
DHI is best for: patients with existing hair in the recipient zone (thinning rather than fully bald), women with diffuse thinning, patients who cannot shave (unshaved DHI), and anyone requiring very high-precision hairline density work.
