Hair Loss in Women —
Causes, Stages & Solutions

Female hair loss is more complex than male pattern baldness. It is rarely just genetics — hormones, thyroid, iron and stress all play a role. Understanding the real cause changes everything about treatment.

Key Difference

Women's Hair Loss Is Not
The Same as Men's

The most important thing to understand about female hair loss: it is rarely a single cause. At Hairmedico, Dr. Arslan requires a blood panel for every female patient before any treatment — because treating the wrong cause leads to failed results.

Men vs Women — How Hair Loss Differs
♂ Men — Androgenetic
  • Primarily DHT-driven, single cause in 95% of cases
  • Receding hairline + crown thinning (Norwood pattern)
  • Progressive and predictable
  • Frontal hairline recedes visibly
  • Blood panel optional
  • Transplant usually straightforward to plan
♀ Women — Multi-factorial
  • Multiple causes: DHT, hormones, thyroid, iron, stress
  • Diffuse thinning across crown (Ludwig pattern)
  • Can fluctuate — not always progressive
  • Frontal hairline typically preserved
  • Blood panel mandatory at Hairmedico
  • Medical causes must be excluded first
Root Causes

Why Women Lose Their Hair —
The 6 Main Causes

40%
Androgenetic Alopecia (FPHL)
Female pattern hair loss follows the Ludwig scale. DHT sensitivity causes diffuse thinning on the crown while the frontal hairline remains largely intact — unlike male pattern baldness.
25%
Hormonal Changes
Menopause, post-partum hair loss, stopping the contraceptive pill, or polycystic ovary syndrome (PCOS) all trigger significant hormonal shifts that affect the hair growth cycle.
15%
Thyroid Dysfunction
Both hypothyroidism (underactive) and hyperthyroidism (overactive) cause hair loss. This is fully reversible with proper thyroid treatment. This is why thyroid function is always checked at Hairmedico before any procedure.
10%
Iron Deficiency & Anaemia
Low ferritin (stored iron) is one of the most common and most frequently missed causes of hair loss in women. Even without anaemia, low ferritin alone can cause significant shedding. Fully reversible with iron supplementation.
5%
Telogen Effluvium
Physical or emotional stress — surgery, illness, extreme dieting, bereavement — pushes hair into the resting phase. Mass shedding occurs 2–3 months later. Almost always reversible once the trigger is resolved.
5%
Autoimmune & Other
Alopecia areata, lichen planopilaris, lupus and other autoimmune conditions. Also: traction alopecia from tight styles, trichotillomania, and medication side effects (chemotherapy, anticoagulants, retinoids).
Life Stages

When Hormonal Triggers
Cause Hair Loss in Women

Female hair loss often coincides with hormonal transitions. Knowing which transition triggered your hair loss helps Dr. Arslan determine whether medical treatment first, or transplant, is the right approach.

Teens – 20s
Contraceptive Pill Changes
Starting or stopping the pill — especially high-androgen pills — can trigger telogen effluvium. Usually reverses within 6–12 months. Early treatment with Minoxidil helps.
Usually reversible
Post-partum
Post-Pregnancy Hair Loss
Up to 50% of women experience significant shedding 2–4 months after childbirth as oestrogen levels drop sharply. Almost always fully reversible by 12 months postpartum.
Fully reversible — wait 12 months
30s – 40s
PCOS & Androgen Excess
Polycystic ovary syndrome raises androgen levels, causing crown thinning similar to male pattern baldness. Anti-androgen medications can slow progression significantly.
Medical treatment first
40s – 50s
Perimenopause & Menopause
Declining oestrogen reduces its protective effect against DHT. Diffuse thinning across the crown becomes progressive. Combined approach: HRT consideration + DHI transplant if stable.
Most common surgical candidate
Any age
Thyroid & Iron Deficiency
Can occur at any age. Often missed for years. Ferritin under 40 ng/mL causes hair loss even without anaemia. Both are fully reversible with treatment — no transplant needed if caught early.
Blood panel always first
Mandatory at Hairmedico
Blood Panel Before Every Female Transplant

"A hair transplant on an undiagnosed thyroid condition or iron deficiency will fail. We check everything before we operate. This is not optional." — Dr. Arslan Musbeh

Ferritin & Iron
Low ferritin causes loss even without anaemia

Treatment Options

How Female Hair Loss
Can Be Treated

Treatment depends entirely on the cause. Medical causes must be treated first. Androgenetic alopecia that is stable and unresponsive to medical treatment is the primary indication for hair transplant in women.

PRP for women
Minoxidil (Topical)
Medical · Ongoing

  • Safe for women — no hormonal side effects
Minoxidil guide

FAQ

Women's Hair Loss
Common Questions

Every woman's hair loss is different. Send photos to Dr. Arslan's team and we will identify the most likely cause and the right next step — free, within 24 hours.

Ask Dr. Arslan's Team
Can women get a hair transplant?+
Yes — women are excellent hair transplant candidates when the cause is androgenetic alopecia and the condition is stable. At Hairmedico, Dr. Arslan uses unshaven DHI for all female patients — no shaving, completely discreet. A blood panel is required first to exclude treatable medical causes.
Does menopause cause permanent hair loss?+
Menopausal hair loss can be permanent if caused by androgenetic alopecia. However, if the primary cause is oestrogen decline, HRT can reduce progression. Once follicles are permanently dormant, a hair transplant is the only restoration option — but medical therapy should be optimised first.
My hair is shedding heavily — should I get a transplant?+
Not yet. Active heavy shedding (telogen effluvium) is a contraindication for hair transplant. The cause must first be identified and treated. Once shedding stabilises — usually 6–12 months — a transplant assessment is appropriate. Dr. Arslan will advise on timing during your free consultation.
Will I need to shave my head for a women's transplant?+
No. Dr. Arslan performs unshaven DHI for all female patients. The procedure is completely discreet — no visible shaving, no obvious signs of surgery. You can return to work within a few days with no one knowing you had a procedure.
Why is a blood panel mandatory for women but not men?+
Because female hair loss has far more potential medical causes than male pattern baldness. Thyroid dysfunction, iron deficiency and hormonal imbalances are common in women and fully reversible with treatment. A transplant on an untreated thyroid condition will partially or fully fail. This blood panel is not optional at Hairmedico — it is a patient protection standard.
What is the Ludwig Scale and how do I find my stage?+
The Ludwig Scale classifies female pattern hair loss into 3 stages based on the width of your central parting and extent of crown thinning. Visit the Ludwig Scale guide to compare your hair to the diagrams — or send photos via WhatsApp for Dr. Arslan's team to assess your exact stage.
