What exactly is shock loss?

When the hair follicle experiences trauma — whether from the surgery itself, from changed blood supply in the recipient zone, or from the disruption of the surrounding tissue — it responds by entering a resting phase called telogen. In this phase, the follicle stops producing new hair shaft growth and the existing hair shaft is shed.

This is the same mechanism as stress-related hair loss (telogen effluvium) that many people experience after illness, surgery or significant stress. The difference with post-transplant shock loss is that it is predictable, universal, and temporary.

Two types of shock loss

Transplanted hair shock loss

All transplanted hairs shed within 2–6 weeks of surgery. This is universal — virtually every hair transplant patient experiences it. The follicle placed in its new location enters telogen while it establishes new blood supply. The shed hair is not a loss — it is a phase. New growth begins from the same follicle at months 3–5.

Native hair shock loss

In some patients, native hairs in or around the transplanted zone also shed temporarily. This is caused by the surgical disruption to the surrounding scalp tissue — altered blood supply, inflammatory response, and physical trauma from channel creation. Native shock loss typically resolves within 3–6 months with complete recovery of the shed native hairs. It is more likely in zones where native hairs are already miniaturised from DHT damage.

What to do during shock loss

Nothing — except follow post-operative care instructions. Shock loss is not a clinical problem requiring intervention. It is not a sign that grafts have failed. It is a phase to be patient through.

The most helpful practical step is to take monthly photographs from the same angle and lighting. At month 3–4, comparing these to the month-1 photo usually reveals that early growth is occurring even when it is not visible in the mirror.

What is shock loss after hair transplant?+

Shock loss (post-transplant telogen effluvium) is the temporary shedding of hair — both transplanted and sometimes native — that occurs 2–6 weeks after surgery. The follicle enters a resting phase after surgical trauma; the hair shaft sheds, but the follicle survives and regrows.

Is shock loss a sign that the transplant failed?+

No. Shock loss is a normal, expected part of the recovery process. The follicles are alive — the shedding is of the hair shaft only. New growth from the same follicles begins at months 3–4.

Does everyone experience shock loss?+

Virtually all patients experience shock loss of transplanted hairs (2–6 weeks post-surgery). Shock loss of native hairs in or near the recipient zone occurs in a minority of patients and also resolves completely within 3–6 months.
