All Articles →Month 2 is the period of maximum patient anxiety — and the period when the least has visibly changed. The shock loss is complete or nearly complete. Transplanted hair has shed. New growth has not yet started. The scalp may look sparser than before the procedure. Patients often contact their clinic convinced the transplant has failed.
This appearance is entirely normal. It is not failure. The follicles are alive beneath the surface — in telogen, the resting phase — recovering from surgical trauma before re-entering anagen. Month 2 is winter. Spring comes at months 3–5.

After surgery, transplanted follicles enter telogen due to the physical and chemical stress of extraction, handling and implantation. The telogen phase typically lasts 8–16 weeks — placing month 2 squarely in the middle of this dormant window. The follicle is intact, alive and cycling. The hair shaft has shed; a new shaft is being prepared for the anagen entry that will occur at months 3–5.
An accurate analogy: a bulb planted in autumn shows no growth in early winter — but the bulb is alive and will emerge in spring. The absence of visible growth at month 2 is biologically identical to this situation.
Normal and expected: No new growth visible. Sparse appearance. Mild residual redness. Some itching as follicles recover.
