Micropigmentation
or Transplant

One adds hair. The other changes how the scalp reads underneath it. They solve different problems and are often used together.

Scalp micropigmentation adds pigment, not hair — it reduces the contrast between scalp and hair so thinning looks denser. A transplant adds real, growing hair. SMP is the option when donor supply is insufficient, when a shaved look is wanted, or when scarring needs camouflage.

When SMP is the better answer

  • Donor supply is insufficient for the coverage wanted
  • You wear or intend to wear your hair very short
  • A donor scar or strip scar needs camouflage
  • Thinning is diffuse and grafts would be spread too thin to read as density
  • You want to reduce contrast without further surgery

The scar case is worth separating out. It is one of the few situations where SMP does something a transplant cannot do well, and it is a common part of the plan after donor overharvesting.

When it is not

SMP does not give you hair to style, and it does not work at longer hair lengths the way it does on a shaved or very short head — pigment dots that read as stubble on a shaved scalp do not read as hair under longer hair. If you want length, a transplant is the only option that provides it.

It also fades. Pigment lightens over years and needs refreshing, and the tone can shift as it does. That is a maintenance commitment, not a one-off procedure.

Using both

The combination is common and often the most practical answer: grafts to rebuild a hairline and add real density, SMP behind and between them to reduce the contrast the eye reads as thinning. Each covers the other's limitation, and the combined result usually looks denser than either alone.
