What actually happens between the surgery and the result — including the shedding phase that alarms almost every patient who was not warned about it.

Transplanted hair sheds before it grows. Grafts fall out between weeks two and four, regrowth starts around months three to four, density becomes visible near month six, and the result settles at roughly twelve to fourteen months. The shedding phase is expected — it is not the graft failing.

Why the shedding phase happens

A transplanted follicle is moved with its blood supply interrupted. It responds by releasing the hair shaft it was growing and entering a resting phase before starting a new cycle. The visible hair goes; the follicle stays. This is why week three often looks worse than the day after surgery, and why judging a transplant before month six tells you nothing.

What helps and what does not

  • Follow the washing protocol exactly — the first ten days protect graft anchorage
  • Sleep elevated for the first nights to limit swelling
  • Avoid direct sun on the recipient area while it is healing
  • Keep to the medication schedule you were given
  • Nothing accelerates the biological cycle; patience through months two and three is the only reliable approach

When does transplanted hair start growing?

New growth typically begins at months three to four, after a shedding phase at weeks two to four. Density becomes clearly visible around month six and the result matures at roughly twelve to fourteen months.

Is it normal for transplanted hair to fall out?

Yes. Shedding between weeks two and four is expected. The hair shaft is released while the follicle stays in place and re-enters its growth cycle. It is not graft failure.

When can I go back to work?

Many patients return within a week, depending on how visible the recipient area is and the physical demands of the job. Redness and crusting are usually the limiting factor rather than discomfort.

Why does my crown look behind the front?

Crown work commonly matures later than the frontal zone. It is normal for the crown to lag by several months.

When can I judge the result?

Not before month six, and not finally before month twelve to fourteen. Assessments made during the shedding phase or the flat months two and three are misleading.
