Applied to Afro hair without modification, standard FUE produces transection rates of 30–80%: the majority of extracted grafts are damaged or destroyed before implantation.

Afro-adapted FUE addresses this through four specific modifications: punch size, entry angle, extraction depth, and graft handling protocol. Each modification targets a different aspect of the curved follicle extraction problem.
Standard FUE uses punches of 0.7–0.8mm diameter. These work well for straight follicles of 0.5–0.7mm diameter — the punch is slightly larger than the follicle and can extract it cleanly.
Afro hair follicles are physically larger — their diameter reflects the larger cross-section of a coiled hair shaft, and the curved root structure requires a wider extraction path. Using a 0.75mm punch on a 0.9mm Afro follicle compresses and damages the follicle wall during extraction.

For straight hair, the punch entry angle follows the visible hair exit angle at the skin surface — which corresponds to the underlying follicle direction. For Afro hair, the visible exit angle does not indicate the follicle direction beneath the surface.

The follicle bends after exiting the skin. A punch inserted to follow the surface exit angle will travel in one direction, while the follicle has already curved in another direction beneath the dermis. Cutting straight down at the surface exit angle reliably transects the follicle at the curve apex.
Dr. Arslan uses a modified entry approach: initial punch insertion matches the exit angle, but depth is limited to 1.5–2mm. Follicle mobilisation then follows lateral oscillation rather than further downward pressure, allowing the punch to follow the natural curve rather than cutting across it.
Successful implantation of Afro grafts requires creating recipient channels that match the expected growth direction and the natural hairline aesthetics of the individual patient. Each channel is created at an angle that allows the coiled hair to emerge at its natural exit angle. Implanting a coiled follicle at the wrong angle produces straight-appearing growth in a section of the hairline — immediately visible and difficult to correct.
