All Articles →Chemical relaxers (lye and no-lye relaxers) work by breaking the disulfide bonds in the hair shaft, permanently straightening the curl. Applied correctly at appropriate intervals, relaxers primarily affect the hair shaft — not the follicle. But misapplication, overprocessing, scalp burns or the combination of relaxers with other chemical or mechanical stress can damage follicles permanently.

The most common patterns of permanent relaxer-related hair loss are: diffuse thinning from repeated chemical trauma to miniaturised follicles; scarring at the scalp where chemical burns have occurred; and combined CCCA progression accelerated by relaxer use.
The majority of relaxer-related hair changes — shaft weakness, breakage, dryness — are not permanent hair loss. They affect the existing hair strand but leave the follicle intact. Discontinuing relaxers and implementing a protein and moisture recovery regime typically restores hair health within 6–18 months.
Permanent loss occurs when the follicle itself is damaged — through scalp burns, severe chemical trauma, or the inflammatory scarring process of CCCA. In these cases, the follicle cannot recover regardless of what products are applied.
Hair transplant is appropriate for relaxer-related hair loss when: trichoscopy confirms permanent follicle loss in specific zones, the overall hair loss pattern is stable, and the donor area is unaffected. This typically applies to women with scalp scarring from chemical burns, women with permanent hairline recession from combined relaxer and traction damage, and women with CCCA that has been accelerated by chemical processing.

Dr. Arslan's assessment for patients with relaxer-related hair loss includes trichoscopy to distinguish permanent follicle loss from shaft damage and temporary shedding, scalp examination for evidence of scarring or ongoing inflammation, and an honest discussion about whether medical management, surgery, or a combination is the appropriate approach. Many patients who present for surgery are better served by a dermatological treatment protocol first — and Dr. Arslan will recommend this when appropriate.
There is no single best age — the best time is when hair loss has been stable for 12+ months. Most surgeons recommend waiting until 25 for the loss pattern to be established. The 30–45 age range is typically optimal: stable pattern, maximum donor density, and realistic long-term planning horizon.
Surgery at 20 is generally not recommended. The hair loss pattern at 20 may not be fully established, and a hairline designed for early loss may look wrong as further recession occurs. Medical management (Finasteride + Minoxidil) should be tried first. Consultation with Dr. Arslan at 20–24 includes honest guidance on timing.
There is no upper age limit. Patients in their 50s, 60s and 70s are excellent candidates — hair loss is stable, donor supply is well-defined, and expectations are realistic. Health determines candidacy more than age at the upper end of the age range.
