Every hair on the human body grows from a follicle — a tube-shaped structure embedded in the scalp. In people with straight hair, this tube runs approximately perpendicular to the scalp surface. In people with Afro-textured hair, the follicle tube follows a curved path — bending at angles of 30–90° beneath the skin before reaching the scalp surface.

This curvature is what produces the characteristic tight coil of Afro hair. But it creates a critical surgical problem: a punch inserted straight down following the visible hair exit angle will not follow the curve of the follicle beneath. It will cut through the follicle at the apex of its curve — destroying the graft entirely.
This is follicular transection, and it is the central technical challenge of Afro hair transplant surgery. In studies using standard FUE techniques on Afro hair, transection rates of 50–80% have been documented. At these rates, a "3,000 graft" session may yield only 600–1,500 viable grafts — a catastrophic outcome that explains many of the poor results reported from inexperienced clinics.

Rather than a single-pass punch to full depth, Dr. Arslan uses a two-stage approach: a shallow scoring incision following the hair's surface exit angle, followed by a curved dissection that tracks the follicle's natural bend. This dramatically reduces the risk of cutting across the follicle mid-shaft.
Every extracted graft is inspected under 10x magnification before storage. Transected grafts — identifiable by their truncated appearance — are recorded and set aside. This allows the team to track viable graft count in real time and adjust extraction pace accordingly.
